Mandatory Occurrence Reporting is an important part of maintaining a safe building environment. It provides a way for safety-related concerns, incidents and potential risks to be identified, reviewed and addressed, helping to prevent similar issues from occurring in the future.

What is this form for?

This form is for reporting any safety related occurrence, incident or other event that could have affected the health, safety or wellbeing of our employees, contractors, clients or members of the public.

Prompt reporting helps us investigate incidents, identify underlying causes, and implement measures to prevent similar events from happening again.

Please provide as much accurate detail as possible. All reports will be reviewed by our HSEQ team and treated in accordance with our reporting and investigation procedures.

Why do we need contact details?

We ask for your contact details so we can investigate the occurrence thoroughly and contact you if we need any additional information. This also allows us to keep you informed of the outcome where appropriate.

Any personal information you provide will be handled confidentially and processed in accordance with applicable data protection legislation. Your details will only be shared with those who need them to investigate and manage the report.

When Should You Make a Report?

You should submit a Mandatory Occurrence Report if you become aware of a potential safety issue that could affect the building or its residents. This may include concerns such as:

  • Defective building work or significant defects
  • Structural safety concerns
  • Fire safety issues
  • Unexpected failures of building materials or systems
  • Any other issue that could present a risk to the safety of people in or around the building

How We Handle Your Report

All Mandatory Occurrence Reports are reviewed and managed by our HSEQ team, who will assess the information provided, determine the appropriate actions required and ensure that any necessary investigations or follow up activities are completed.

We will keep you informed where appropriate and use the information provided to help identify risks, improve safety measures and prevent similar issues from occurring in the future.

All reports are handled confidentially and in line with data protection requirements. Details of the person who submitted a report will only be shared with those who need the information to manage the issue.

How To Make a Report

If you identify a potential safety concern, please contact the relevant Divisional Manager using their details below. They will be able to discuss the issue with you, gather any relevant information and ensure that it is reviewed through the appropriate process.

When reporting an issue, please provide as much detail as possible, including what you have noticed, where it has occurred and any supporting information that may help us investigate.
